Pro-renin is the precursor form of renin, an enzyme that plays a crucial role in the renin-angiotensin-aldosterone system (RAAS), which regulates blood pressure, fluid and electrolyte balance, and systemic vascular resistance. Pro-renin is synthesized and secreted by the juxtaglomerular cells of the kidney. Once in the bloodstream, it can be activated to renin, which then cleaves angiotensinogen to produce angiotensin I. Angiotensin I is subsequently converted to angiotensin II, a potent vasoconstrictor, by the angiotensin-converting enzyme (ACE). Angiotensin II has multiple effects, including increasing blood pressure, stimulating aldosterone secretion from the adrenal glands, and promoting sodium and water reabsorption by the kidneys. Elevated levels of pro-renin and renin are associated with hypertension, heart failure, and chronic kidney disease. Understanding the regulation and function of pro-renin is essential for developing therapeutic strategies aimed at modulating the RAAS pathway to treat cardiovascular and renal diseases. Pro-renin and its receptor also represent potential targets for novel drug development in the management of these conditions.
